Requirements:

Languages: Candidates must have knowledge of the English Language
Education: Candidates should have completion of post secondary education in Divinity
Experience: Candidates should have considerable experience in the relevant field

Physical Requirements:

  • The candidates should be able to work independently and be self-motivated, self-directed while possessing the ability to work in a collaborative manner
  • The candidates should have the ability to work flexible hours if required, including workdays, evenings, and weekends and travel to various long-term care homes across the City

Other Requirements:

  • The candidates should have or a related field, or an equivalent combination of education and experience
  • The candidates should have experience providing spiritual care in a fast-paced healthcare and long-term care environment, preferably within a large, complex unionized organization or environment and current membership with the Canadian Association for Spiritual Care (CASC) or its Canadian or American equivalent and successful completion of Clinical Pastoral Education (CPE)
  • The candidates should be able to relate theology and behavioral science to the practice of spiritual care
  • The candidates should demonstrate spiritual maturity with a high degree of sensitivity, compassion, and gentleness and knowledge of critical incident responses and the ability to facilitate crisis intervention
  • The candidates should have strong interpersonal, verbal and written communication, and organizational skills
  • The candidates should have excellent problem-solving skills with the ability to apply sound judgment and innovative solutions to challenging issues and demonstrated experience in collaborating with teams and fostering positive relationships with internal and external stakeholders
  • The candidates should have knowledge of and utilization of the principle of continuous quality improvement is Proficiency with Microsoft Office applications (spreadsheets, Word, Excel, PowerPoint, internet and literature searches, e-mail) and Point Click Care
  • The candidates should have experience working with diverse populations and communities Indigenous, Black, 2SLGBTQI+, and other equity-deserving groups)

Responsibilities:

  • The candidates should be able to implement detailed plans and recommend policies or procedures changes regarding program-specific requirements and provide input into the assigned budget in consultation with the Manager, ensuring that expenditures are controlled and maintained within approved budget limitations and collaborate with the Homes’ management team to provide support and short-term counseling to residents, families, and staff following traumatic events and as needed to ensure their health and wellbeing. They should make appropriate referrals and maintain documents or records
  • The candidates should be able to monitor and address performance issues of external spiritual and religious service providers and handle sensitive or confidential information, which could require investigation of resident, family, and staff; document evidence and report to the immediate Supervisor or Manager for further action
  • The candidates should be able to develop, coordinate, and maintain spiritual and religious care programs for residents and their families and coordinate the completion of admission assessments and residents’ participation in the legislated care conference, if applicable. They should respond to all referrals and document all spiritual/religious care visits, care planning, and quarterly Resident Assessment Instrument-Minimum Data Set (RAI-MDS)
  • The candidates should be able to monitor and chart resident interactions, collaborating with the interprofessional care team to develop a plan of care or advance directives related to goals of care discussions for palliative and end-of-life care residents and assess the spiritual and religious care needs of residents and offer care directly and make appropriate referrals to other members of the interprofessional team and community partners
  • The candidates should be able to collaborate and work in partnership with the Coordinator of Volunteer Services and volunteers to develop a network of faith-based community partners; develop roles and responsibilities; provide onboarding training and monitor external spiritual and religious volunteers and coordinate worship services including prayer and memorials and ensure a schedule of regular worship services is posted at each Home area. They should assist in coordinating special bereavement functions such as Celebration of Life and Memorial Services
  • The candidates should be able to provide spiritual and religious care support for residents and family members within the continuum of care, particularly for those experiencing spiritual and emotional distress and deliver religious education programs to pastoral care volunteers, if applicable, in collaboration with the Manager of Resident Services and the Coordinator of Volunteer Services. They should provide staff with practical education on religious faiths to assist them in understanding residents’ and families’ spiritual
  • The candidates should be able to utilize community resources to meet the spiritual needs of multi-faith residents and families, ensuring that residents have the opportunity to contact the faith community of their choice and coordinate the Ministry of Faith Groups providing services in SSLTC and facilitate contact with residents’ former Faith Groups, when applicable
  • The candidates should be able to participate in quality improvement processes related to spiritual and religious care and collect, monitor, and report relevant risk management data and perform functions related to spiritual and religious care in the Home(s) in consultation with the Manager of Resident Services and/or Coordinator of Volunteer Services
  • The candidates should be able to provide reports and maintain ongoing communication with the Manager of Resident Services regarding administrative and operational matters in the delivery of spiritual and religious care and participate in Home committees, as defined by SSLTC policy, and raise spiritual and religious care issues within the interprofessional care team and provide opportunities for prayer, sacraments, and other spiritual, religious, or cultural rituals
  • The candidates should be able to establish and maintain a list of residents by faith group and ensure residents have access to spiritual, religious, and cultural rituals and services as appropriate or requested and contribute to the development of policies, procedures, and protocols related to spiritual and religious care, end-of-life, and palliative care under the Resident Services portfolio
