Requirements:

Languages: Candidates must have knowledge of the English Language
Education: Candidates should have current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice
Experience: Candidates should have experience as a Nurse Practitioner in long-term care, acute care, rehabilitation, continuing care, or primary care

Other Requirements:

  • The candidates should be able to incorporate knowledge of diversity, cultural safety, and the determinants of health in the assessment, diagnosis, and therapeutic management of residents and the evaluation of outcomes
  • The candidates should have the ability to exercise independent judgment and assess situations and problems efficiently and effectively

  • The candidates should have the ability to provide clinical supervision, education, and mentoring for nursing students, medical students, and other learners, including residents/families

  • The candidates should have strong and effective verbal and written communication skills

Responsibilities:

  • The candidates should be able to deliver primary health care to residents in the Long-Term Care Home, focusing on the management of acute and chronic medical conditions, therapeutic management, health promotion, and disease/injury prevention in order to deliver comprehensive health services
  • The candidates should be able to conduct focused health assessments using and adapting assessment tools and techniques based on resident needs
  • The candidates should be able to complete health history documentation, incorporating physical, psychosocial, emotional, cultural, and ethnic dimensions of health
  • The candidates should be able to perform physical examinations, identify normal and abnormal findings, and interpret them accurately
  • The candidates should be able to order diagnostic investigations and interpret the results using evidence-based clinical reasoning
  • The candidates should be able to synthesize health information, employing critical inquiry and clinical reasoning to formulate diagnoses, assess health risks, and determine states of health/illness
  • The candidates should be able to communicate effectively with residents, discussing health assessment findings and/or diagnoses, including outcomes and prognosis, drawing upon knowledge of pathophysiology, psychopathology, epidemiology, infectious diseases, behavioral sciences, and family processes
  • The candidates should be able to select appropriate interventions from a range of non-pharmacological and pharmacological options to restore or maintain residents’ functional, physiological, and mental stability, striving for optimal health
  • The candidates should be able to promote safe resident care by identifying and mitigating harm, addressing immediate risks for residents, and responding to adverse events and near misses
  • The candidates should be able to collaborate with the Medical Director and other senior management staff in developing and implementing evaluation processes, as well as identifying strategies to continuously improve clinical outcomes and effectively manage risks
  • The candidates should be able to collaborate with senior management in the Division and other community health services, participating in initiatives that promote continuity of resident care across the continuum
  • The candidates should be able to ensure that daily practice aligns with the standards set by Accreditation Canada
  • The candidates should be able to maintain proper documentation and maintenance of resident files and health care records, ensuring compliance with applicable legislation and divisional expectations
  • The candidates should be able to identify and collaborate with the Medical Director on educational programs for the care team
