Requirements:

Languages: Candidates must have knowledge of the English Language

Experience: Candidates should have four years of current clinical practice including social work, counseling, crisis intervention, suicide risk assessments, case conferencing, planning, management with adults, children and families

Physical Requirements:

  • The candidates should expect to provide aspects of their assigned duties in clients’ home environments, which may constitute 30-50% of their work hours. This includes driving, standing, and performing client assessments or care while bending, reaching, pulling, and standing in awkward positions or cramped spaces
  • The candidates should understand that while most of their work hours will be in the Home Care office, there are no unusual environmental demands and understand that their work schedule is Monday to Friday during regular working hours (0800-1630) and prepared to spend most of their work hours in the Home Care office, where there are no unusual physical demands
  • The candidates should be willing to see clients independently in their homes in the community, which could make up to 30-50% of their work hours. This involves working alone in unpredictable, unsecured, and sometimes unpleasant conditions that must be managed independently
  • The candidates should be aware of potential exposures in clients’ homes, such as unsanitary conditions, cigarette smoke, pets, loud noises, extremes of heat and cold, communicable diseases, infectious organisms, needle stick injuries, blood and bodily fluids, cytotoxic medications and waste, and other hazardous materials
  • The candidates should be capable of driving to client homes, transferring in and out of vehicles, and navigating potentially unsafe walkways and stairs in various weather conditions, ranging from -40 to +30 degrees Celsius, including wind, rain, snow, and insects or bug
  • The candidates should anticipate dividing their day between direct client care, case management, and providing support to staff and prepared for outdoor walking in winter conditions for approximately 7 months of the year and potentially visiting clients outside Yellowknife city limits where phone service may be limited or unavailable
  • The candidates should note that while most of their work hours in the Home Care office involve no unusual sensory demands, visiting clients in their homes (up to 30-50% of work hours) requires acute cognitive focus and the combined use of touch, sight, smell, and hearing for assessment and care provision in uncontrolled settings
  • The candidates should be prepared for potential distractions in client homes that may affect normal assessment and therapeutic communication, such as noise, interruptions, pets, and visual distractions and possess effective communication skills to handle potentially difficult interactions with clients, including those who may be volatile, difficult, intoxicated, or verbally and physically abusive
  • The candidates should be willing to develop relationships with Home Care Program clients and demonstrate the ability to remain calm, controlled, and professional in various situations, including supporting peaceful and dignified deaths and managing emotionally stressful situations
  • The candidates should demonstrate the ability to provide ethical care while maintaining appropriate professional boundaries, even in cases where close therapeutic relationships may develop and committed to continuous motivation and innovation in continuing education and practice to support their professional growth and maintain psychosocial fitness for providing quality care to clients and staff

Other Requirements:

  • The candidates should possess knowledge of healthcare concerns and challenges to effectively identify and address health issues impacting clients and their families
  • The candidates should demonstrate proficiency in applying psychosocial processes such as assessment, planning, coordination, facilitation, and evaluation
  • The candidates should have the ability to teach and communicate effectively with clients and colleagues from diverse age groups and cultures, using appropriate English language skills to understand and respond to client needs and collaborate in the client’s best interests
  • The candidates should exhibit strong interpersonal and communication skills, both verbal and written, to interact effectively with clients, their families, community resources, outside agencies, and other healthcare professionals
  • The candidates should possess awareness of biological, physical, and behavioral sciences to recognize, interpret, and prioritize health issues and implement action plans based on accepted standards of practice
  • The candidates should demonstrate flexibility to ensure effective and efficient use of resources and maintain professional peer relationships
  • The candidates should be capable of functioning within an interdisciplinary team setting, actively participating in morning huddles, staff meetings, and team or case conferences to promote teamwork practices
  • The candidates should be self-directed, with the ability to meet deadlines, prioritize workload, and manage multiple projects and requests simultaneously
  • The candidates should exhibit sound judgment and creative problem-solving skills, enabling them to make decisions independently while interacting with acute and chronically ill clients with changing
  • The candidates should possess excellent listening skills and maintain a non-judgmental attitude and knowledge of the community and its available resources

Responsibilities:

  • The candidates should be able to assess the client’s social and psychosocial status and specific needs in view of their care requirement and provide ongoing follow-up and evaluation to determine the efficacy of interventions and coordinate and prioritize caseload effectively
  • The candidates should be able to promote effective communication to facilitate information sharing between clients, staff, physicians, other healthcare providers, and outside providers or agencies
  • The candidates should be able to work collaboratively with HCC team to provide services to seniors and those living with disabilities, advocating for their financial, educational, and personal care
  • The candidates should be able to assess the emotional, psychosocial, and environmental needs of clients and coordinate necessary resources to meet those  and document client’s needs assessment and develop a service plan with projected goals and intervention planning
  • The candidates should be able to collaborate and consult with clients, their families, interdisciplinary teams, community agencies, and other resources to support discharge and follow-up
  • The candidates should be able to educate clients and families to develop an understanding of how service providers, clients, and families work together to achieve desired goals and collaborate with interdisciplinary HCC team members, health, social, and community agencies regarding client care
  • The candidates should be able to provide case management and act as a liaison to acquire necessary resources to facilitate a client’s successful reintegration into the community
  • The candidates should be able to set priorities and establish goals of care that are responsive to the health and social needs and preferences of clients, families, the home setting, and cultural context
  • The candidates should be able to maintain familiarity with and establish working relationships with health services, continuing care programs, community counseling programs, other health and social services professionals, and community/territorial agencies to advise clients and families of available resources
