Requirements:

Languages: Candidates must have knowledge of the English Language
Education: Candidates should have Current certificate of Competence from the College of Nurses of Ontario
Experience: Candidates should have experience working in a long-term or acute care environment

Physical Requirements:

  • The candidates should provide a satisfactory Police Reference Check (PRC) with vulnerable sector screening, tuberculosis screening and proof of immunization in accordance with the Long-Term Care Homes Act (O.Reg.79/10)
  • The candidates should be capable of working in a cooperative manner with other staff members in the coordination of resident care
  • The candidates should be able to provide nursing care to ambulatory and non-ambulatory residents with a focus on restoration, health promotion, and palliation regardless of complexity and predictability
  • The candidates should be physically capable of lifting, turning, and transferring residents manually and with a mechanical lift
  • The candidates should have the ability to provide management presence in the Home during off-hours as the Registered Nurse in Charge

Other Requirements:

  • The candidates should have Baccalaureate in Nursing or a certificate in gerontology or the approved equivalent combination of education and experience as per Ontario Regulation 79/10 made under the Long-Term Care Homes Act,20077
  • The candidates should be knowledgeable in nursing theory, biological and social sciences, applying them to the standard established by provincial nursing legislation and knowledge of the current principles of gerontology and accepted best practices where appropriate
  • The candidates should possess knowledge of community resources and demonstrate a proven ability to coordinate outside services to support resident independence and ability to perform holistic assessments/reassessments for residents, regardless of complexity, to establish a comprehensive plan for care and identify the appropriate category of care provider
  • The candidates should be capable of recognizing, analyzing, and interpreting deviations from predicted resident response to care/treatment, responding by modifying the plan of care using professional judgment and autonomous decision-making
  • The candidates should demonstrate the ability to evaluate overall resident outcomes and the achievement of goals and prioritize and organize work activity, providing work direction to staff members to complete assignments
  • The candidates should have the ability to apply an analytical decision-making approach, drawing on a comprehensive range of options to interpret, analyze, and solve problems
  • The candidates should possess the ability to educate residents and their families in death and wellness strategies, promoting restorative care, prevention of disease, and knowledge of good health habits to assist in restoration
  • The candidates should have excellent verbal and written communication skills and able to support the Toronto Public Service values to ensure a culture that champions equity, diversity, and respectful workplaces
  • The candidates should be able to evaluate theoretical and research-based approaches for application to practice and proficient in utilizing computers and various software packages such as Microsoft Office

  • The candidates should demonstrate the ability to develop, coordinate, monitor care delivery, and evaluate care plans that will promote resident independence and participation in decision-making

Responsibilities:

  • The candidates should be able to develop, evaluate, and modify care plans for residents and provide nursing care, including counseling, instruction, and the coordination of resident care, such as activation and recreation
  • The candidates should be able to direct staff by assigning duties and establishing priorities and examine, measure, and record temperature, pulse, respirations, blood pressure, serum levels of glucose, weight, and neurological assessments using equipment such as a blood pressure cuff, glucometer, stethoscope, ophthalmoscope
  • The candidates should be able to administer and interpret results of mental status tests using the Folstein Test for cognitive recognition and assess the physical, emotional, and psychological well-being of residents/clients through observation and analysis of test and physical health indicators to evaluate care plans and identify modifications that may be required
  • The candidates should be able to accompany physicians on rounds, providing information on resident/client condition and progress, and assisting with examinations, such as positioning, dressing, and undressing residents/clients and able to transcribe verbal or written physician’s orders for medication or other treatment
  • The candidates should be able to prepare precise dosages of medication in accordance with physician’s orders, including narcotic and controlled drugs, and administer them to residents/clients and able to maintain inventory, ensuring medications are properly stored, safe, and secure
  • The candidates should be able to administer prescribed treatments as ordered by the physician, such as catheterizations, respiratory therapies, suctions, exercise, wound irrigations, and hot pack therapies using electronic/manual equipment for treatments, such as a suction machine, hydrocollator
  • The candidates should be able to administer first aid and emergency health measures, such as CPR and check resident/client diet for therapeutic composition and able to prepare and maintain documentation such as charts, nursing records, statistics, and reports
  • The candidates should be able to prepare and apply clean and sterile dressings and bandages, and replace them as required. They should also clean wounds and lesions, remove stitches and clips, and practice sterile techniques and other infection control procedures to minimize the transmission of infection
  • The candidates should be able to monitor and participate in the personal care of residents/clients, including hygiene, feeding, positioning, transferring, ambulation, and toileting, using electric/hydraulic/manual equipment and lifting devices
  • The candidates should be able to liaise, refer, and advocate with community resources to coordinate admittances, transfers, and departures/deaths, involving hospitals (acute care, rehabilitation, and palliation), public health department, home care program, long-term care facilities, ambulance services, diagnostic services, the coroner’s office, and social services
  • The candidates should be able to perform delegated Medical Acts (duties) such as vaccinations, inoculations, eye irrigations, and death pronouncements and able to counsel, instruct, and confer with residents/clients and their families in planning care, providing emotional support, responding to inquiries, resolving complaints, and train student nurses
