Requirements:
Languages: Candidates must have knowledge of the French and English Language
Education:Â Candidates should have completion of a B.Sc. in Pharmacy and be registered with the Ontario College of Pharmacists (Part A)
Experience: Candidates should have at least two years of pharmacy experience
Physical Requirements:
- The candidates should possess the ability for self-directed learning and independent working
- The candidates should be able to work effectively with individuals from diverse backgrounds
Other Requirements:
- The candidates should have experience working within a mental health setting or possess a Hospital Pharmacy Residency and a postgraduate pharmacy degree
- The candidates should have knowledge of a computerized unit dose drug distribution system and in-depth knowledge of pharmacy operations and medication-use processes
- The candidates should understand the clinical and distributive activities associated with medication use and have knowledge of medication safety risks in health care
- The candidates should possess analytical skills, including the ability to assess and take responsibility for patient drug-related needs and offer value-added solutions
- The candidates should have excellent communication (both oral and written) and interpersonal skills to work effectively with the patient population
- The candidates should demonstrate strong organizational skills as well as problem recognition and problem-solving skills
- The candidates should have bilingualism (French and English) and proficiency in a second language
Responsibilities:
- The candidates should be able to provide a full range of clinical and distributive pharmacy services to inpatients and outpatients in a computerized environment
- The candidates should be able to apply clinical expertise in psychiatry and addictions to ensure safe and effective medication management for patients
- The candidates should be able to collaborate with pharmacy team members and healthcare providers to optimize patient care
- The candidates should be able to evaluate medication orders, develop pharmaceutical care plans, and resolve drug therapy problems
- The candidates should be able to collect best possible medication histories and facilitate medication reconciliation
- The candidates should be able to ensure accurate dispensing of pharmaceuticals within an interdisciplinary team setting
- The candidates should be able to participate in risk management, patient safety, clinical knowledge sharing, and quality improvement initiatives
- The candidates should be able to teach, mentor, and transfer knowledge to students, residents, and interns, support the department’s commitment to education and research
- The candidates should be able to foster a workplace that embraces diversity and complies with all applicable regulatory and legislative requirements
