Requirements:

Languages: Candidates must have knowledge of the French and English Language
Education: Candidates should have completion of a B.Sc. in Pharmacy and be registered with the Ontario College of Pharmacists (Part A)
Experience: Candidates should have two years of pharmacy experience

Physical Requirements:

  • The candidates should have the ability to work effectively with individuals from diverse backgrounds

Other Requirements:

  • The candidates should have experience working within a mental health setting or possess a Hospital Pharmacy Residency and a postgraduate pharmacy degree
  • The candidates should have knowledge of a computerized unit dose drug distribution system and in-depth knowledge of pharmacy operations and medication-use processes
  • The candidates should have an understanding of the clinical and distributive activities associated with medication use and knowledge of medication safety risk in health care
  • The candidates should have analytical skills, including the ability to assess and take responsibility for patient drug-related needs to offer value-added solutions
  • The candidates should possess excellent communication (both oral and written) and interpersonal skills to work effectively with the patient population
  • The candidates should have demonstrated organizational skills as well as problem recognition and problem-solving skills and ability for self-directed learning and working
  • The candidates should have bilingualism (French or English) and proficiency in a second language

Responsibilities:

  • The candidates should be able to work collaboratively with other pharmacy team members and health care providers
  • The candidates should be able to evaluate medication orders, develop pharmaceutical care plans, and resolve drug therapy problems
  • The candidates should be able to collect best possible medication histories and facilitate medication reconciliation
  • The candidates should be able to ensure accurate dispensing of pharmaceuticals in an interdisciplinary team setting
  • The candidates should be able to support and participate in risk management, patient safety, clinical knowledge sharing, and quality improvement activities
  • The candidates should be able to teach, mentor, and transfer knowledge to students, residents, and interns, while supporting the department’s commitment to education and research
  • The candidates should be able to support a workplace that embraces diversity and complies with all applicable regulatory and legislative requirements
