Eastern Health’s Community Support Services department provides essential care and assistance to individuals within the community. It offers a range of support services such as home care, meal delivery, and transportation assistance. Through personalized care and community outreach, the department aims to enhance the well-being and independence of individuals. Requirements:

Languages: Candidates must have knowledge of the English Language
Education: Candidates should have Graduation from a recognized/approved Bachelor’s Degree in Social Sciences/Humanities or Business
Experience: Candidates should have considerable experience in the relevant field

Physical Requirements:

  • The candidates should regularly move/lift objects less than 10 lbs. and occasionally provide assistance to lift objects up to 30 lbs with push/pull light to medium forces for patient handling
  • The candidates should be constantly seated and perform fine finger precision work when using a computer and should regularly stand, walk, and drive to client visits
  • The candidates should maintain alertness and concentration while frequently completing repetitive tasks and maintaining constant auditory/visual concentration

Other Requirements:

  • The candidates should have knowledge of financial assessments for home supports, medical discharge, long-term care, assistance, and/or subsidy programs
  • The candidates should possess knowledge of community support agencies, programs, and eligibility requirements
  • The candidates should have knowledge of complex health concerns and should exhibit strong advocacy skills
  • The candidates should demonstrate strong critical thinking, problem-solving, and time management skills
  • The candidates should demonstrate the ability to work independently and should demonstrate strong oral and written communication skills
  • The candidates should have strong interpersonal skills with clients/patients/families and various community resources
  • The candidates should know the use of a personal vehicle and a Class 5 driver’s license and have a satisfactory record of work performance

Responsibilities:

  • The candidates should be able to provide resource counseling and social support services to clients within the Community Supports Program
  • The candidates should be able to work as an integral member of interdisciplinary teams and closely collaborate with each team member
  • The candidates should be able to engage in significant independent clinical practice in conjunction with Social Worker case managers
  • The candidates should be able to report to the Division Manager and fulfill their primary responsibilities accordingly
