Requirements:

Languages: Candidates must have knowledge of the English Language
Education: Candidates should have completion of post-secondary education in the social services
Experience: Candidates should have experience conducting risk assessments with participants and their family members

Physical Requirements:

  • The candidates should be legally entitled to work in Canada, provide and maintain a satisfactory Criminal Record Check with Vulnerable Sector Search
  • The candidates should have a valid Full Class 5 Manitoba driver’s licence, access to a vehicle, and the ability to travel within the Province of Manitoba for government business
  • The candidates should be able to provide and maintain a satisfactory Child Abuse Registry Check, provide and maintain a satisfactory Adult Abuse Registry Check, work on a regular rotation, including evenings, weekends, and statutory holidays
  • The candidates should have excellent time-management and organizational skills, including the ability to manage high-volume caseloads and competing demands within a complex and fast-paced work environment
  • The candidates should be able to work independently and exercise initiative within a multi-disciplinary team

Other Requirements:

  • The candidates should have case management experience in an integrated services environment that includes brief counselling, conducting assessments, service planning and coordination, resource development, and follow-up
  • The candidates should have experience working with children with disabilities or vulnerable adults and their families, providing supports based on the individual needs of the participants
  • The candidates should have experience with crisis intervention and protection services, effective verbal communication skills, strong written communication skills
  • The candidates should have strong interpersonal skills, including the ability to interact sensitively, respectfully, and in a non-judgmental manner to resolve conflict effectively with diverse stakeholders
  • The candidates should be proficient in Microsoft Office Suite (Word, Excel, and Outlook) or similar software applications

Responsibilities:

  • The candidate should be able to lead and actively participate in Service Coordination teams, engaging a variety of government and community agencies across sectors such as health, education, child welfare, and justice
  • The candidate should be able to act as a liaison between individuals, families, advocates, and external agencies to ensure coordinated and person-centered support services,  deliver services using a family-centered and person-centered approach, applying discipline-based strategies and interventions tailored to client
  • The candidate should be able to assess, plan, implement, and monitor individualized service plans for children, families, and vulnerable individuals
  • The candidate should be able to manage crisis situations, provide support services, and coordinate substitute decision-making processes as per applicable legislation Adults Living with an Intellectual Disability Act)
  • The candidate should be able to advocate on behalf of clients, coordinate transitions to childcare, schools, residential or day programs, and counsel individuals facing complex social challenges
  • The candidate should be able to build networks of support that help clients manage and overcome issues such as trauma, disability, abuse, addiction, mental health challenges, homelessness, and family breakdown
  • The candidate should be able to collaborate with professionals, represent CLdS/CdS to stakeholders and community members, and promote the independence and potential of every client served
