Requirements:

Languages: Candidates must have knowledge of the English Language
Education: Candidates should have educational qualifications such as a Bachelor of Social Work or Registered Nurse
Experience: Candidates should have experience providing case management

Physical Requirements:

  • The candidates should have exposure to pets
  • The candidates should have exposure to second-hand smoke, latex products, flowers and/or plants, and other irritants while in client homes or within facilities
  • The candidates should Travel within Whitehorse

Other Requirements:

  • The candidates should have experience developing person-centred care plans
  • The candidates should have experience completing assessments in their respective field. Strong ability to collaborate and work within a multi-disciplinary team
  • The candidates should possess excellent organizational, interpersonal, and conflict management skills
  • The candidates should have effective oral and written communication skills
  • The candidates should demonstrate strong skills working with various cultural backgrounds
  • The candidates should have the ability to take a reflective approach to their practice
  • The candidates should have undergone TB screening and hold a vulnerable sector RCMP security clearance
  • The candidates should possess a valid Class 5 Driver’s Licence and WHMIS 2015 certification
  • The candidates should maintain current registration with a Canadian regulatory body

Responsibilities:

  • The candidates should be able to provide person-centered care coordination to all residents
  • The candidates should be able to ensure care is holistic, continuous, culturally sensitive, and compassionate, and values resident/family experiences
  • The candidates should be able to approach health care as a partnership between the health care provider and resident, where the resident’s assessed needs, values, priorities, and goals guide the care planning and implementation processes, and they are an equal partner in the planning of care
  • The candidates should be able to ensure every action performed and decision made is founded on respect for the residents, for promotion of their dignity, individual freedom, choice, and lifestyle, and meaningful, quality living
