Requirements:
Languages: Candidates must have knowledge of the English Language
Education: Candidates should have successful completion of an approved Nurse Practitioner program and a minimum of master’s degree
Experience: Candidates should have considerable experience in the nursing field
Other Requirements:
- The candidates should be registered with CRNA on the Nurse Practitioner Registrar, including Provisional Graduate Nurse Practitioner licensure
- The candidates should hold current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP) certification
- The candidates should demonstrate the ability to provide support and advice to Continuing Care teams, encompassing care planning, assessment, care interventions, and program development for CC clients with complex cognitive/mental health/behavioral concerns, palliative care needs, complexities of chronic illness, co-morbidities, and frailty
- The candidates should exhibit the ability to actively integrate Client and Family-Centered-Care principles into practice
- The candidates should possess excellent communication and interpersonal skills to collaborate effectively with members of the inter-professional team
- The candidates should have proven experience in critical thinking, conflict resolution, and problem-solving skills
- The candidates should also hold current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP) certification
Responsibilities:
- The candidates should be able to conduct comprehensive assessments and provide consultative services to thirteen physicians
- The candidates should be able to, as a leader and mentor for the team, assist with the more unique and complex transitions from Acute Care to home or residential care
- The candidates should be able to participate on a Nurse Practitioner Advisory Committee and collaborate with Nurse Practitioners in Grande Prairie and our Palliative Care team
- The candidates should be able to provide primary care or help patients navigate their finances or social matters
- The candidates should be able to provide a full range of comprehensive health services to individuals across their lifespan
- The candidates should be able to be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients
- The candidates should be able to utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems
