Requirements:

Languages: Candidates must have knowledge of the English and French (oral) Languages.
Education: Candidates need standard educational qualification such as a Bachelor’s Degree in Pharmacy or Pharm D
Experience: Candidates need experience in providing exceptional patient centered care and also having experience in hospital pharmacy systems and distribution systems are an asset.

Other Requirements:

  • The candidate should have a current registration as a licensed Pharmacist with the Ontario College of Pharmacists, Part A and also having completed a hospital pharmacy residency or equivalent experience is an asset
  • The candidate should have demonstrated interpersonal, problem solving, organizational and communication skills and also be self-motivated and able to take initiative
  • The candidate having membership in the Canadian Society of Hospital Pharmacists is an asset

Responsibilities:

  • The candidate should be able to report to the Pharmacy Supervisor and work as an integral member of the Pharmacy team and be accountable for the pharmacotherapy for patients to ensure optimal patient care is achieved as well as embody a culture of caregiver inclusion in the patient’s plan of care
  • The candidate should be able to evaluate evidence-based medicine to support the recommendations and to develop improvement strategies for drug utilization
  • The candidate should be able to identify, prevent, and resolve patients’ drug therapy problems through the development and implementation of a therapeutic plan and also maintain professional conduct and standards in all aspects of his/her work and when interacting with hospital staff and the public
  • The candidate should be able to interpret, assess and where necessary, clarify the physicians’ medication orders and also participate in the education of patients and their families regarding drug therapy where appropriate.
  • The candidate should be able to check and verify dispensing and compounding activities of pharmacy technicians when needed, including oncology systemic therapy orders and preparations and also participate in hospital committees as assigned
  • The candidate should be able to share the evening and week-end coverage
