Requirements:
Languages: Candidates must have knowledge of the French and English Language
Education: Candidates should have completion of a B.Sc. in Pharmacy and be registered with the Ontario College of Pharmacists (Part A)
Experience: Candidates should have a minimum of two years of relevant pharmacy experience
Physical Requirements:
- The candidate should have demonstrated organizational skills, problem recognition and problem-solving skills
- The candidate should possess the ability for self-directed learning and independent working
- The candidate should have the ability to work effectively with individuals from diverse backgrounds
Other Requirements:
- The candidate should have experience working within a mental health setting or possess a Hospital Pharmacy Residency and a postgraduate pharmacy degree
- The candidate should have knowledge of a computerized unit dose drug distribution system
- The candidate should have an in-depth knowledge of pharmacy operations and medication-use processes
- The candidate should have an understanding of clinical and distributive activities associated with medication use
- The candidate should have knowledge of medication safety risks in health care, strong analytical skills, including the ability to assess and take responsibility for patient drug-related needs and offer value-added solutions
- The candidate should possess excellent communication (both oral and written) and interpersonal skills to work effectively with the patient population
- The candidate should have bilingualism (French and English) and proficiency in a second language
Responsibilities:
- The candidate should be able to provide a full range of clinical and distributive services to inpatients and outpatients within the context of a computerized environment, apply clinical expertise in psychiatry and addictions to provide safe and effective medication management to patients of CAMH
- The candidate should be able to work collaboratively with other pharmacy team members and health care providers, evaluate medication orders, develop pharmaceutical care plans, and resolve drug therapy problems
- The candidate should be able to collect best possible medication histories and facilitate medication reconciliation, ensure accurate dispensing of pharmaceuticals in an interdisciplinary team setting
- The candidate should be able to support and participate in risk management, patient safety, clinical knowledge sharing, and quality improvement activities
- The candidate should be able to teach, mentor, and transfer knowledge to students, residents, and interns, support the department’s commitment to education and research
- The candidate should be able to support a workplace that embraces diversity and complies with all applicable regulatory and legislative requirements
