Requirements:

Languages: Candidates must have knowledge of the English Language
Education: Candidates should have completion of Master’s degree in a recognized Social Science program with addiction-specific education
Experience: Candidates should have considerable experience in the relevant field

Other Requirements:

  • The candidates should have a Bachelor’s in a recognized Social Science program with addiction-specific education
  • The candidates should be a member in good standing with the relevant regulatory college or governing body in Ontario (OCSWSSW, COTO, CNO) or in pursuit of relevant credentials (Registered Psychotherapist – RP)
  • The candidates should be competent in the provision of CBT, DBT, and other evidence-based approaches for treating complex mental health conditions and substance use disorders
  • The candidates should have demonstrated skills and experience in assessment and treatment using evidence-based practices
  • The candidates should have awareness of the value of measurement-based care when supporting patients or clients
  • The candidates should possess exceptional communication and interpersonal skills and the ability to facilitate and lead an interdisciplinary team, orchestrating team resources
  • The candidates should have individual and group counselling skills, including the ability to present psycho-education to large groups and the capacity to deliver inclusive and culturally responsive approaches
  • The candidates should have knowledge of addictions and concurrent issues along a continuum of care and hold a valid Ontario driver’s license and have access to their own vehicle
  • The candidates should have knowledge of the Mental Health Act, PHIPA, the Ontarians with Disabilities Act 2005, and the Health Care Consent Act

Responsibilities:

  • The candidates should be able to coordinate, initiate, and conduct assessments to help guide treatment planning
  • The candidates should be able to provide case management services, including service planning, coordination, navigation, and evaluation of progress
  • The candidates should be able to provide individual and group psychotherapy using evidence-informed practices such as Motivational Interviewing, Stage-Matched Intervention, Trauma-Informed approaches, Brief Solution-focused Therapy, CBT, and DBT-informed approaches, including CPT and other evidence-based practices where applicable
  • The candidates should be able to offer individual and group support to family members and lead group counseling and education sessions
  • The candidates should be able to work closely with Physicians and the clinical team to monitor the patient experience through ongoing meetings to review progress, create and adjust treatment plans, and liaise with referral sources
  • The candidates should be able to take responsibility for developing and implementing discharge planning and referrals to appropriate community resources
  • The candidates should be able to identify discharge needs based on comprehensive patient assessments and care plans
  • The candidates should be able to provide service and team leadership by establishing treatment goals and collaborating with patients and team members to develop actionable steps to meet these goals
  • The candidates should be able to lead team monitoring and evaluation of progress on treatment plans and act as a communication lead among patients, the team, and the program, ensuring critical communications occur internally and externally
  • The candidates should be able to complete documentation and record-keeping in compliance with Homewood Health Centre and governing College policies
