Requirements:
Languages: Candidates must have knowledge of the English Language
Education: Candidates should hold a minimum of a master’s degree
Experience: Candidates should have a minimum of 1 year of Seniors Health experience
Physical Requirements:
- The candidates should have a valid driver’s license and access to a motor vehicle in good repair for work-related travel, and be able to travel in inclement weather
- The candidates should be able to carry a 30lb medical bag and various equipment
Other Requirements:
- The candidates should have successfully completed an approved Nurse Practitioner program
- The candidates should be registered with CRNA on the Nurse Practitioner Registrar, including Provisional Graduate Nurse Practitioner licensure
- The candidates should have current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP) certification
- The candidates should have a minimum of 5 years of clinical experience as a Nurse Practitioner
- The candidates should have a minimum of 1 year of clinical experience managing palliative care clients in an advanced practice role (within the last 3 years)
- The candidates should have experience in at least two (2) care sectors, including Home Care and possess advanced clinical and assessment skills
- The candidates should be competent with MS Office Suite, TEAMs, ZOOM, and tele-health platforms
- The candidates should have CNA Certification in Palliative Care and completed McMasters Case Management Education
- The candidates should be trained in Meditech and Connect Care systems and previous experience in a leadership role
- The candidates should have experience in program planning, education of adults, interdisciplinary practice, and research activities
Responsibilities:
- The candidates should be able to provide a full range of comprehensive health services to individuals across their lifespan
- The candidates should be able to integrate clinical skills from nursing and medicine to assess, diagnose, and interpret diagnostic tests
- The candidates should be able to prescribe treatments and perform procedures to manage and meet patients’ health
- The candidates should be able to utilize advanced nursing practice competencies in Clinical Practice, Leadership, and Health Systems Optimization
- The candidates should be able to promote care quality, appropriate use of health services, and influence healthcare systems through education and research
