Requirements:

Languages: Candidates must have knowledge of the English Language
Education: Candidates should have completion of Bachelor’s in nursing
Experience: Candidates should have considerable experience in the relevant field

Physical Requirements:

  • The candidates should demonstrate willingness to travel by road and aircraft
  • The candidates should expect overnight travel as part of this position to provide service to clients in communities
  • The candidates should be capable of lifting up to 15kg, which may include audiovisual and clinical supplies and equipment
  • The candidates should be prepared for exposure to emotionally distressed patients and families in isolated clinical settings
  • The candidates should be a registered nurse in good standing with a Canadian registered nursing regulatory body
  • The candidates should have the ability to work autonomously and also within a multi-disciplinary team environment

Other Requirements:

  • The candidates should have strong knowledge and experience in nursing best practices, standards and chronic disease management through case management of clients with chronic conditions
  • The candidates should have excellent oral and written communication, analytical, problem-solving, and organizational skills
  • The candidates should have an excellent ability to initiate relationships with clients, their families, and other health professionals
  • The candidates should have the ability to provide direction and deliver services in a competent, safe, and ethical manner
  • The candidates should have strong engagement, facilitation, and presentation skills
  • The candidates should be proficient in the use of computers and software, including MS Teams, Word, Excel, and PowerPoint
  • The candidates should have experience facilitating the learning of others through workshops, training, or teaching
  • The candidates should possess Vulnerable Sector Security Clearance
  • The candidates should have undergone TB Screening, CPR-HCP, registration with the Yukon Registered Nurses Association (YRNA) or equivalent certification
  • The candidates should hold a valid Class 5 driver’s license or equivalent and certification as a Respiratory Educator and Certified Diabetes Educator

Responsibilities:

  • The candidates should be able to provide quality, planned, proactive, person-centered chronic disease management within the Chronic Conditions Support Program (CCSP)
  • The candidates should be able to conduct client assessments and determine chronic conditions risk factors
  • The candidates should be able to provide clinical care, education, self-management support, and caregiver support and education
  • The candidates should be able to collaborate as a member of the CCSP team in a clinical setting
  • The candidates should be able to empower clients and families to self-manage chronic conditions
  • The candidates should be able to support program goals through the provision of high-quality, collaborative chronic care, and effective and accessible self-management support
