Requirements:
Languages: Candidates must have knowledge of the English Language
Education:Â Candidates should have baccalaureate degree in nursing with strong clinical background in nursing
Experience: Candidates should have 3 years home care experience within the last five years’ management and and experience relevant to nursing practice in home care
Physical Requirements:
- The candidates should demonstrate a clear pattern of professional and personal development
- The candidates should demonstrate the ability to identify potential gaps in care and work collaboratively to ensure client needs are met, delivering exceptional and individualized care
- The candidates should demonstrate the ability to collaborate with interdisciplinary team members, including but not limited to acute care
- The candidates should have the ability to prioritize and achieve results in a fast-paced and changing environment
- The candidates should exhibit excellent organizational and time management skills, be self-directed, and demonstrate the ability to work independently using sound professional judgment
- The candidates should have the ability to provide effective education and client teaching
Other Requirements:
- The candidates should have possession of a valid driver’s license and access to a motor vehicle in good repair for work-related travel and current license or active registration from the appropriate professional body
- The candidates should hold a current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP) certification and strong background in recruitment along with experience with RMS (Recruitment Management System)
- The candidates should possess a working knowledge of relevant community resources along with transitional resources
- The candidates should have proven leadership skills, including team building, resource management, conflict resolution, and written and verbal communication, as well as facilitation/change management
- The candidates should be solution-oriented with a focus on decision-making and resolving complex issues and have geriatric experience/certification with training in case management principles
- The candidates should possess knowledge of the continuing care sector and excellent interpersonal and communication skills
- The candidates should demonstrate critical thinking and decision-making skills.
The candidates should have strong assessment skills - The candidates should have experience in wound care, palliative care, and the provision of home parenteral therapy
- The candidates should have computer knowledge/skills, specific to the Meditech application, Microsoft Word, and Outlook
Responsibilities:
- The candidates should be able to handle day-to-day operational duties of managing a Home Care office, including, but not limited to: mentorship and development of staff, assisting with staff scheduling, addressing client concerns, and implementing and maintaining standards of practice
- The candidates should be able to work closely with community partners, both contracted and non-contracted, to ensure home care clients can remain in their home for as long as possible
- The candidates should be able to demonstrate good time management, critical thinking abilities, and the capability to work both independently and within a team environment to promote client-centered care
