Requirements:

Languages: Candidates must have knowledge of the English Language
Education: Candidates should have completion of bachelor’s degree in Social Work
Experience: Candidates should have minimum 2 years of experience in the relevant field

Other Requirements:

  • The candidates should be active or eligible for registration with the Alberta College of Social Workers (ACSW) and possess a Driver’s License and a vehicle, as they are required
  • The candidates should demonstrate a high degree of independence, critical thinking, and problem-solving skills, along with an ability to prioritize workload
  • The candidates should possess a reliable vehicle and a valid Driver’s License and Previous experience working in the healthcare system
  • The candidates should have completed provisional registration requirements and possess excellent written and verbal communication skills
  • The candidates should have knowledge of relevant legislative Acts and experience with guardianship, trusteeship, personal directives, and capacity assessment processes
  • The candidates should have knowledge and experience working with Income Support programs at community, provincial, and federal levels
  • The candidates should have knowledge and ability to use Microsoft Office, Outlook, and Teams and have experience working within an interdisciplinary team

Responsibilities:

  • The candidates should be able to work both independently and within multidisciplinary teams to provide family and patient-centered care across acute care and community settings
  • The candidates should be able to provide psychosocial services using a collaborative approach to support clients, families, and health professionals across the continuum of care, including acute care, home care, supportive living, and long-term care
  • The candidates should be able to conduct supportive counseling, resource-based interventions, psychosocial assessments, comprehensive risk assessments, cognitive screening, and support transitions and discharge planning
  • The candidates should be able to provide education to clients and their families on substitute decision-making, available resources, and supports within and external to Alberta Health Services
  • The candidates should be able to deliver a range of social work services to patients and their families, including psychosocial assessment, resource-based interventions, individual, group, and family treatment, risk screening, counseling, case management, coordination, transition or discharge planning, and prevention, promotion, and public awareness activities
