Requirements:
Languages: Candidates must have knowledge of the English Language
Education:Â Candidates should have a post-secondary degree or diploma in relevant discipline (e.g., BSW, MSW, OT, RN)
Experience: Candidates should have a personal experience with mental health and/or addiction and established recovery, as well as willingness to share experience
Other Requirements:
- The candidates should understand the experiences and needs of the mental health and addictions populations
- The candidates should be able to build rapport and initiate conversations with others using interpersonal skills
- The candidates should approach client care non-judgmentally and utilize trauma-informed best practices
- The candidates should understand the importance of confidentiality and boundaries
- The candidates should be able to conduct assessments and coordinate services that are recovery-oriented based on need
- The candidates should have fluency in a second language, including ASL (American Sign Language), is considered an asset
- The candidates should have strong interpersonal skills with a proven ability to approach, listen, build trust, and communicate across diverse cultures and intersectionality. They should also have a willingness to continually improve understanding around cultural competence
- The candidates should be willing to learn and apply practical understanding of different elements of diversity, such as gender identity/expression, race/cultural background, religion, sexual orientation, disability, immigration and precarious status, family status, etc.
- The candidates should be open to operating from an anti-racist, anti-oppressive, 2SLGBTQ+ affirming, and intersectional framework
- The candidates should provide proof of administered full-series Health Canada approved COVID-19 vaccination prior to the start date. If unable to be vaccinated for reasons relating to protected grounds under the Code, completed medical or religious exemption must be provided
- The candidates should possess a valid Canadian driver’s license and have access to an insured automobile in good repair
- The candidates should provide a satisfactory Vulnerable Sector Screening (Police Check) report
- The candidates should have experience and/or training in Wellness Recovery Action Plan (WRAP) and Concurrent Disorders
- The candidates should have experience and/or training in Cognitive Behavioural Skills and Motivational Interviewing
- The candidates should have experience and/or training in Trauma Informed Care and Virtual Care
- The candidates should have experience and/or training in Interprofessional Multi-Disciplinary Teams and Crisis Intervention Skills
Responsibilities:
- The candidates should be able to provide holistic assessment and case management using a stepped care approach
- The candidates should be able to carry an individual caseload in addition to participating in team-based decision-making and care planning
- The candidates should be able to conduct assessments and assist clients to develop goals for recovery using one-to-one and group sessions
- The candidates should be able to utilize tools such as the Wellness Recovery Action Plan (WRAP) to assist clients in creating individual care plans
- The candidates should be able to support clients in developing empowerment skills and combating stigma through self-advocacy
- The candidates should be able to assist in identifying and combatting negative self-talk and identifying and overcoming fears by sharing own techniques and strategies
- The candidates should be able to manage a caseload including regular reassessments, updating risk levels, and caseload reviews
- The candidates should be able to collaborate with the multi-disciplinary team to build a client care plan and provide short-term or intensive case management as needed
- The candidates should be able to facilitate groups as part of CMHA PD’s multi-disciplinary team, including the creation of new groups and act as a back-up facilitator
- The candidates should be able to refer and link clients to community resources and groups that support recovery goals
- The candidates should be able to participate in regular staff meetings and care planning to provide the peer expertise for client care
- The candidates should be able to contact primary care at regular intervals to report clients’ care plan and discharge summary
- The candidates should be able to work collaboratively with other Peer Support Workers to support organizational needs (e.g., volumes across all regions)
- The candidates should be able to update all appropriate documentation within 24 hours of client contact, as required by CMHA PD policy and procedure
- The candidates should be able to participate in the Peer Community of Practice and other agency and community committees and workgroups as requested by the manager
