Requirements:

Languages: Candidates must have knowledge of the English Language
Education: Candidates should have post secondary degree (or diploma) and be registered as a Social Worker with a Canadian Social Work regulatory authority
Experience: Candidates should have experience working in a Health Care Environment

Physical Requirements:

  • The candidates should have exposure to animals
  • The candidates should have exposure to second hand smoke from clients/residents
  • The candidates should have exposure to latex products and plants/flowers
  • The candidates should be willing to travel within the territory
  • The candidates should demonstrate the ability to work independently and within a team setting

Other Requirements:

  • The candidates should have experience doing RAI-HC assessments and the provision of home care and/or long-term care services
  • The candidates should be familiar with the Care Consent Act and provision of “Capability to Consent to Care in a Care facility”
  • The candidates should demonstrate the ability to develop, implement, and evaluate plans for clients with complex care
  • The candidates should possess excellent interpersonal, conflict management, communication skills and assessment skills
  • The candidates should have the ability to provide case management or care coordination in a larger healthcare system
  • The candidates should hold current Canadian registration as a registered nurse with the Yukon Registered Nurses Association if applying as an RN, or as a Physiotherapist with the Yukon Registrar of Physiotherapists, Social Worker, or Occupational Therapist with registration/licensing from a Canadian from a Canadian occupational therapist regulatory authority, or a Physiotherapist (PT) registered
  • The candidates should possess Vulnerable Sector RCMP security clearance
  • The candidates should undergo TB screening
  • The candidates should hold CPR level C certification and WHMIS 2015 certification
  • The candidates should possess a valid class 5 driver’s license

Responsibilities:

  • The candidates should be able to assess client referrals into Continuing Care programs
  • The candidates should be able to perform standardized intake assessments
  • The candidates should be able to determine the appropriate level of care for clients
  • The candidates should be able to identify risks and determine priorities of placement
  • The candidates should be able to coordinate admissions into programs/services within Continuing Care
  • The candidates should be able to provide primary case management up to the admission date
