Requirements:

Languages: Candidates must have knowledge of the English Language
Education: Candidates should have a minimum of a Bachelor’s Degree or equivalent (RN, SW, OT)
Experience: Candidates should have experience in Mental Health and Addictions is required, preferably in a clinical community setting

Other Requirements:

  • The candidates should possess solid knowledge and understanding of mental health and addictions assessment and treatment, including mental status examinations, intensive case management (ICM), and legislative and regulatory requirements relevant to ICM
  • The candidates should exhibit a professional, respectful attitude in line with recovery principles and HPS Philosophy, demonstrating sensitivity to the diversity and uniqueness of HPS clients
  • The candidates should demonstrate clinical reasoning skills by employing evidence-informed reasoning, sound decision-making, and problem-solving abilities
  • The candidates should showcase effective listening, verbal, and non-verbal communication skills with clients and multidisciplinary team members, as well as proficiency in documentation
  • The candidates should have a proven track record of developing therapeutic relationships, possessing case management ability, and proficient documentation skills in clinical health records
  • The candidates should be responsible, accountable, and focused on quality management
  • The candidates should possess a valid driver’s license, access to a reliable vehicle, and insurance
  • The candidates should hold a valid First Aid & CPR/AED Level C certification
  • The candidates should have computer literacy, particularly with Office 365, and the ability to learn new programs
  • The candidates should hold experience and training in Cognitive Behavior Therapy, Dialectical Behavior Therapy, and Motivational Interviewing are assets
  • The candidates possess bilingualism (French/English) or proficiency in a second language is an asset

Responsibilities:

  • The candidates should be able to deliver intensive case management and recovery-oriented psychosocial rehabilitation support to individuals with schizophrenia and schizoaffective disorder
  • The candidates should be able to partner with clients, working together to achieve goals, in a dynamic and supportive team environment
  • The candidates should be able to support individuals with the development and implementation of an individualized, comprehensive recovery-oriented plan based on the client’s goals and needs
  • The candidates should be able to develop a therapeutic connection with clients
  • The candidates should be able to assess and monitor client mental status
  • The candidates should be able to assess, develop a plan, and implement support and programs that facilitate the client’s recovery
  • The candidates should be able to monitor, evaluate, and document progress and outcomes
  • The candidates should be able to advocate and link to other resources as required
  • The candidates should be able to work collaboratively with the HPS team
  • The candidates should be able to provide associate case manager and office ‘triage coverage’ duties, as assigned
  • The candidates should be able to administer medication by injections to clients of HPS (nurse only)
  • The candidates should be able to participate in medication stock and supply management (varying roles depending on discipline)
  • The candidates should be able to participate in clozapine monitoring system (varying roles depending on discipline)
  • The candidates should be able to participate in the education of health professional students
  • The candidates should be able to participate in program evaluation activities
  • The candidates should be able to maintain clinical documentation at appropriate professional standards
  • The candidates should be able to adhere to the administrative and clinical guidelines described in the HPS Policies and Procedures
  • The candidates should be able to document workload measurement
  • The candidates should be able to perform other duties as assigned
