Requirements:
Languages: Candidates must have knowledge of the English Language
Education:Â Candidates should hold an undergraduate degree in a discipline from a recognized university
Experience: Candidates should have 3 to 5 years of case management experience with individuals and groups facing concurrent mental health and substance use challenges
Physical Requirements:
- The candidates should demonstrate proficiency in trauma-informed and strength-resiliency-based assessment, psychosocial, and brief counseling techniques
- The candidates should work effectively with multi-disciplinary teams and be proficient in using computers and various software applications
Other Requirements:
- The candidates should have including those who inject drugs, face homelessness, or have criminal justice involvement
- The candidates should have 3-5 years of experience working with diverse populations, such as people who use drugs, racialized communities, Indigenous peoples, individuals engaged in sex work, prisoners/ex-prisoners, people with mental health challenges, and trauma survivors
- The candidates should demonstrate proficiency in completing case management assessments and organizing and leading case conferences
- The candidates should have experience in mentorship and delivering training programs and extensive knowledge of referral pathways for internal and external resources
- The candidates should possess skills in advocacy, crisis intervention, prevention, conflict resolution, and negotiation
- The candidates should exhibit extensive knowledge of various systems (CTS’s, SOS, RAAM, CAS, PGT, OW/ODSP, criminal justice, taxes, immigration, legal, housing, transportation, food security) and the ability to navigate them with professionalism and excellent written and oral communication skills
Responsibilities:
- The candidates should be able to build rapport with clients and collaborate with community and clinical care teams to enhance connectivity to health and social care through mobile Harm Reduction Case Management
- The candidates should be able to provide time-limited, focused care to improve access to safer supply alternatives and Harm Reduction Clinical Care for clients
- The candidates should be able to conduct comprehensive assessments, coordinate care and follow-up, and facilitate community integration for clients
- The candidates should be able to provide system advocacy and connect clients to long-term supports as appropriate
- The candidates should be able to foster collaborative partnerships to ensure rapid integration of critical health services during periods of transition
- The candidates should be able to monitor, modify, and strengthen client support networks to empower self-determination and sustainable independence following care transitions
