Requirements:

Languages: Candidates must have knowledge of the French and English Language
Education: Candidates should have completion of a Bachelor’s degree in Social Work, Psychology, Nursing, or a related health sciences field
Experience: Candidates should have two or more years’ clinical experience working directly with individuals with mental illness, addictions, and complex comorbidities

Physical Requirements:

  • The candidates should have the ability to work with patients from diverse cultural and social backgrounds

Other Requirements:

  • The candidates should show proof of registration and be a member in good standing within their respective College if in a regulated health care discipline
  • The candidates should demonstrate bilingualism (French and English) as a requirement for this position
  • The candidates should possess excellent organizational, communication, and interpersonal skills to manage complex relationships with community stakeholders in a self-directed manner and as part of a dynamic interprofessional team
  • The candidates should have experience working with community-based providers, providing effective leadership, chairing intake and clinical meetings, and demonstrating strong decision-making, organizational, and problem-solving skills
  • The candidates should have both hospital experience and community-based work experience with individuals with mental illness, addictions, and/or complex comorbidities
  • The candidates should have experience collecting data for evaluation and research purposes
  • The candidates should demonstrate computer literacy and be familiar with computer-based programs, including MS Office, Outlook, and Excel

Responsibilities:

  • The candidates should be able to monitor, triage, and process referrals effectively while scheduling sessions and documenting participant progress
  • The candidates should be able to tailor session topics to meet the unique needs of participants
  • The candidates should be able to consult and collaborate weekly with an interprofessional Virtual Care Team (VCT) to ensure comprehensive participant care
  • The candidates should be able to assist participants in navigating appropriate care providers and resources
  • The candidates should be able to coordinate and facilitate educational group sessions on an as-needed basis
  • The candidates should be able to work collaboratively with an interdisciplinary team of internal and external stakeholders, including healthcare providers, research staff, and people with lived experience
  • The candidates should be able to conduct environmental scans, develop resources, prepare project updates, and contribute to writing manuscripts
  • The candidates should be able to adhere to Equity, Diversity, and Inclusive Principles in clinical treatment and research while being respectful, ethical, culturally sensitive, and mindful of research and evaluation practices
  • The candidates should be able to support a healthy workplace by embracing diversity, encouraging teamwork, and ensuring compliance with all applicable policies, regulatory, and legislative requirements
  • The candidates should be able to work collaboratively with internal and external stakeholders to achieve project deliverables and foster strong partnerships and work at 1025 Queen Street West and accommodate evening hours
